This form may be required for your reservation and includes our agreements on a written contract so that both parties can acknowledge their responsibilities. Please sign and return to [email protected] after you have submitted your retainer/deposit. Both the retainer and the contract must be signed in order to secure your appointment. The General Policies Affirmation applies to all photo sessions and packages.
